I just unlocked The Lost with help of a new reddit thread: TerraPony ( talk) 21:57, 3 December 2015 (UTC) Afterbirth Azazel Seed The Holy Mantle can only be unlocked as a STARTING item in the Afterbirth DLC. On the first paragraph of the text, it says that the item "Holy Mantle" requires the recent Afterbirth expansion pack, wich, it doesn't, using as reference youtube videos from before the release of the expansion, and the Holy Mantle page itself.
